Active Pathways is currently seeking dedicated Bank Support Workers to work across Park Place (Supported Living Service) and Millcroft (Step-Down Residential Rehabilitation Service) in Seacroft, Leeds. This is a fantastic opportunity for a passionate individual who wants to make a real difference in the lives of the residents within their care.

Key Responsibilities

  • Support service users in achieving their life goals, promoting independence and enhancing their quality of life.
  • Develop, implement, and maintain person-centered support plans tailored to individual needs.
  • Handle challenging behavior with professionalism, demonstrating resilience and problem-solving skills.
  • Administer medication in accordance with guidelines, ensuring compliance and safety.
  • Provide care and support during lone working shifts, ensuring the safety and well-being of the residents.
  • Build and sustain trusting relationships, offering guidance and support to help residents lead full, happy, and healthy lives.

Experience & Skills

  • Previous experience within a Mental Health or Rehabilitation setting.
  • Familiarity with support plans and medication administration.
  • Strong understanding of challenging behaviors and how to handle them effectively.
  • Ability to work independently, showing resilience and initiative, especially in lone working scenarios.
  • A collaborative approach to working within a multidisciplinary team.

About the Role

Active Pathways is looking for a dedicated Registered Mental Health Nurse (RMN) or Registered Nurse in Learning Disabilities (RNLD) to join the team at Brookhaven, a 23-bedded open-rehabilitation service for adults with enduring mental health needs. This is a fulfilling and varied role, offering the opportunity to work collaboratively with individuals to improve their quality of life and achieve their goals.

Brookhaven focuses on inclusion, skill development, risk reduction, and relapse prevention through a robust Recovery and Social Inclusion Programme, connecting residents to leisure activities, educational opportunities, and vocational work schemes.

Requirements:

  • Active NMC PIN and a degree in Mental Health or Learning Disabilities Nursing
  • Experience working within mental health or rehabilitation settings, including medication training

  • Familiarity with current legislation and the importance of least restrictive practice
  • Someone who is resilient, organised, and approachable

Active Pathways is looking for an experienced Band 6 Occupational Therapist to join our team at Heathy House, a specialist rehabilitation service supporting individuals with complex mental health needs. This role also covers two additional services in Leeds, offering a varied and dynamic caseload.

About the Role:
As a Band 6 OT, you'll play a key role in supporting recovery and promoting independence through high-quality interventions. Your responsibilities will include:

  • Delivering tailored one-to-one and group therapies to enhance daily living skills and social inclusion.
  • Working closely with a multidisciplinary team to create personalised care plans.
  • Conducting comprehensive assessments to support service users' recovery journeys.
  • Leading the development of OT pathways and contributing to service improvements.
  • Travelling across multiple sites to provide support where needed. Multi-site travel will be ad-hoc and based on the requirements of the service user, ensuring that care is delivered where it is most needed at the time.

About the Services:

  • Heathy House is a 10-bed rehabilitation service that provides a step-down residential service.
  • Park Place is a purpose-built supported living development.
  • Millcroft is a purpose-built step-down residential rehabilitation service.

What We’re Looking For:

  • A qualified OT with a Diploma/Degree in Occupational Therapy and HCPC registration.
  • At least 3 years of post-graduate experience, with 2+ years in mental health settings.
  • A proactive professional who thrives in a fast-paced environment and enjoys a varied caseload.
  • Strong communication skills and the ability to work collaboratively with colleagues and service users.
  • Experience mentoring students or junior staff is desirable.
  • driving license and access to a car is required, as multi-site travel will be involved, paid at 45p per mile.
